Usefulness of Spontaneous Speech Analysis Scales in Patients with Mild Cognitive Impairment and Dementia of Alzheimer¡¯s Type

Abstract
Objectives: Various scales have been used for analyzing spontaneous speech, but there has been no comparative study on spontaneous speech analysis scales for patients with mild cognitive impairment (MCI) and dementia of Alzheimer¡¯s type (DAT). This study aims to examine the usefulness of spontaneous speech analysis scales in patients with MCI and DAT.

Methods: Thirty elderly with normal cognition, 30 patients with MCI, and 30 patients with DAT participated in this study. The Boston Cookie-Theft picture description task was used and analyzed for the ratio of Correct Information Unit (CIU)-morphological words, the ratio of CIU-syntactic words, main concepts, and the number of content words by utterance.

Results: The results from this study are as follows. First, the normal cognition group had significantly higher performances only in the number of content words by utterance compared to the MCI group. Second, the MCI group had significantly higher performances only in the ratio of CIU-syntactic words than the DAT group. Third, the normal cognition group had significantly higher performances among all spontaneous speech analysis scales than the DAT group. Fourth, the number of content words by utterance was a useful scale for patients with MCI, and the ratio of CIU-syntactic words was a useful scale for patients in the DAT group as shown by results of the ROC curve analysis.

Conclusion: The present study suggests that different spontaneous speech analysis scales are useful in discriminating among normal cognition, MCI and DAT in clinical practice.
